Referral

Thank you for referring your patient to Milestones Pediatric & Maternal Nutrition, LLC.

For nutrition counseling, we are currently in-network with Aetna, Anthem, BCBS (not including Blue Home or Blue Local), Cigna, Medcost, NC Medicaid (including all Medicaid Managed Care plans), UMR, & United HealthCare and able to accept HSA & FSA cards.

For lactation counseling, we are currently in-network with Aetna and partnering with The Lactation Network to provide insurance coverage for families with Anthem, BCBS, Cigna, Multiplan, Provider Network of America (PNOA), and United Healthcare plans and able to accept HSA & FSA cards.

Please take a moment and complete either the letter of medical necessity form (required for FSA/HSA coverage) or the referral form (required for health insurance coverage). Once completed, please fax the completed form along with pertinent medical records and insurance information (as appropriate) to 919-899-0667.
